Scoring Service Reminders
Please ensure that you are familiar with these steps when using our Scoring
Service for the CTBS, CCAT, and GMRT:
ORDERING IDENTIFICATION SHEETS
Please order and send back the proper Identification Sheets (I.D.'s).
For efficient service, specify the number of buildings and the number
of classes in each building.
For each order you will require:
- one class I.D. sheet for each class tested
- one building I.D. sheet for each school tested
- one instruction sheet for each order (may be reproduced), including
detailed instructions
- one Order for Scoring Service Form (may be photocopied from Nelson's
Scoring Service Handbook)
ORGANIZING ANSWER SHEETS
- Inspect student answer sheets and I.D. sheets. Marks must be dark
- the letter or number should not show through the "answer oval." Marks
should be done with an HB pencil and answered according to the directions
in the teacher's guide. There should be no marks or writing outside
designated areas.
- Stack the answer sheets for each class with the student Name block
facing up. Arrange the answer sheets in the order in which you wish
them to appear on the reports.
- Place a completed Class/Group I.D. Sheet on top of each set of answer
sheets. All Identification Sheets must be filled out with an HB pencil.
- Stack the classes for each school by grade, combine the grades, and
place a completed Building I.D. Sheet on top.
- If there is more than one school, combine answer sheets by building.
- Fill out the photocopiable Scoring Service Order Form. Keep a photocopy
of this Order Form for your files.
- Do not mix Tests or forms. Each Test or form is treated as a separate
package.
- There is a minimum charge of $100 to $150 per scoring-service package,
for any one form of each test, depending on services ordered.
PACKAGING AND SENDING ANSWER
SHEETS
- Package all materials securely so that sheets do not move about inside
the package. Forward to the Scoring Service. Lost or damaged answer
sheets are invariably the result of poor or flimsy packaging.
- DO NOT USE staples, elastics, paper clips, or pins. A good
practice is to enclose each school's sheets in an envelope marked ___
of ____ (i.e., 1 of 2, 2 of 2, etc.), and package securly in a sturdy
carton.
- If more than one carton is required, label cartons number ___ of ___,
and include the Order for Scoring Service in carton One.
NOTE: It is best to send the answer sheets in a way that can be traced by
the Post Office or shipping company, should the parcel get lost. In all
cases, you are urged to use the fastest method of shipment that is economically
feasible. Unless otherwise specified, results will be returned in the same
manner as received.
Please be advised that Nelson assumes no responsibility for shipments lost
en route from the school to the Scoring Service.

Turn-around Time: 10 working days
(exclusive of transportation). Peak scoring periods occur in November,
May, and June.
Note: Labour charges will be applied if the above directions are not followed.
Most often these charges are for:
- Replacing missing or photocopied Identification Sheets
- Repairing answer sheets damaged by paper clips, elastics, etc.
- Cleaning up stray marks, particularly those in the black timing bars
along the side of the answer sheets. Please note that problems with
the condition of the answer sheets can result in delays in the processing
and in the turn-around time for reports.
- There is a minimum charge of $100 to $150 per scoring-service package,
for any one form of each test, depending on services ordered.
- Prices are effective January 2, 2007.
